Job Posting: TLC Program Manager
Transitional Level of Care (TLC) Program Manager: Winthrop University Hospital is seeking an individual with clinical and administrative experience to manage our TLC program. Experience in Geriatrics, Skilled Nursing Facility and Home Care with quality improvement and data management skills is necessary. The TLC Program Manager will report to the Medical Director of the TLC Program and to the VP for Continuum of Care. The manager will work with inpatient care teams, case managers and act as liaison to post acute care facilities. S/he will develop processes and workflows to facilitate identification of hospitalized patients requiring enhanced post acute care monitoring. The TLC Program Manager will identify hospitalized patients for whom continuity of care and communication is critical and will coordinate post hospital care with other providers including disease/syndrome clinical teams, Sub acute care services, Home Care, and others. Clinical background required. NP or RN and others with demonstrated experience in nursing home, home care and/or other post acute care preferred.
