The Division of Geriatrics is seeking to hire a fellowship trained geriatrician to join our full time faculty. We are an active division with five geriatricians, two nurse practitioners and ACGME accredited fellowship program. We are currently expanding our faculty practice to a new state of the facility and developing innovative programs with local nursing homes. We are seeking to hire a geriatrician who is enthusiastic about providing care in multiple settings. This position includes a competitive base salary, full benefits and a faculty practice plan.

Experience in Geriatrics, Skilled Nursing Facility and Home Care is preferred. The TLC Program Manager will report to the Medical Director of the TLC Program and to the VP for Continuum of Care. The manager will work with inpatient care teams, case managers and liaison at post acute facilities. S/he will develop relationships, processes and workflows to facilitate identification of patients requiring enhanced post acute care monitoring.

The TLC Program Manager will identify patients for whom continuity of care and communication is critical and will coordinate post hospital care with other providers including disease/syndrome clinical teams, Sub acute care services, Home Care, and others.
